New Horizons - Children's Community Nursing Team

The New Horizons team consists of experienced children’s nurses and support workers who provide care to children aged 0-18 in their own homes, school or care setting. The team delivers care to children who have specific nursing needs. Children need to be under the direct care of a consultant paediatrician and be registered with a Gloucestershire G.P

Services That Are Provided

• Direct nursing care – including dressing changes, feeding support, intravenous therapy, injections, renal support and home respiratory support
• Management and coordination of complex care packages agreed with continuing health care
• Education and health promotion
• Support for children and their families who require palliative care
