“Supporting Care Homes to achieve excellence in Care”
The Care Home Support Team is a team of therapists, general nurses, mental health nurses and pharmacists providing support and training to care home staff.
The team provides education, training and hands on support to homes to enable care assistants, cooks, activity coordinators and nurses to provide the best possible care and quality of life to individual residents.
The team comprises of:
3 General Nurses
5 Mental Health Nurses
2 Mental Health Trainers
Medicines Management Team
Occupational Therapist
Physiotherapist
Speech and Language Therapist
Dietitian
Podiatrist
The Care Home Support Team was originally set up by the Partnership for Older People Projects (POPP), and was a partnership project between Gloucestershire County Council, Gloucestershire Primary Care Trust and other independent organisations, including Gloucestershire Older Persons’ Assembly (GOPA), Gloucestershire Care Providers Association (GCPA), and Gloucestershire Neighbourhood Projects Network.
It is now fully funded by NHS Gloucestershire and Gloucestershire County Council.
On this website, we have aimed to provide information to enable care home staff to book training and signpost to the various services that are available to them.
